Tirzepatide has produced larger average weight reductions in clinical trials and is often considered for patients with more weight to lose, or for those who have plateaued on a single-pathway GLP-1. Semaglutide has the longest track record and the deepest published evidence base, which makes it a strong starting point for many patients.
Tolerance and prior response.
Gastrointestinal side effects vary between individuals and between compounds. A patient who struggled to tolerate one may do considerably better on the other, which is why titration and follow-up matter more than the initial selection.
Authenticity of supply.
